Creating a new page for the form

Step 1

Once you have logged in (go to How to get to the Edit Interface for help), click on the New button from the top menu.

Step 2

In the text box in the menu bar, type a title for the new page.
Click on the Next button.

Step 3

A blank page will then be created and shown.
