Here's what information is needed in the above fields...

Channel Request Date: The date this request is submitted.
Proposed Channel Name: What would you like to name this channel?
Channel Category: What category will this channel be listed under?
Channel Requestor Name: Who is requesting this channel?
Channel Requestor Email: What is the channel requestor's email address?
Channel Requestor Phone: What is the channel requestor's phone number?
Channel Owner Name: Who is responsible for making updates and modifications to this channel?
Channel Approval: Who approves of this channel request? Who agrees that this channel may be developed? Who signs-off that this channel has been developed as expected?
Channel Purpose: What is the channel's "mission statement"? (What is the purpose of this channel?)
Channel Objectives: What are this channel's goals and objectives?
Channel Description: Brief description of channel appearance, functionality, basic look-and-feel.
Channel Content Source: Where will this channel's content come from (existing web page, etc.)? Does the content already exist? If so, please provide a link to it.
Channel Content Type: What is the data type of this channel's content? (e.g. Static HTML, XHTML, dynamic content, XML, etc.?)
Channel Content Owner: Who is responsible for providing this channel's content?
Channel Content Workflow: What are the procedures for updating channel content?
Channel Content Update: How often will content be updated?
Channel Needed-by Date: By what date should this channel be completed?
Channel Publish Date: By what date should this channel be in production?
Channel Stale Date: What date will this channel be deactivated?
Channel Priority: What is the priority of this channel?
Channel Dependencies: What dependencies exist for developing this channel?
Channel Roles / Subscribable: What roles may access this channel, or will it be subscribable to all users?
Channel Status: What is the current development status of the channel?
Channel Clone? Is this channel essentially identical to any existing channel? If so, which one?
Channel Type: Will this be a targeted content channel, RSS feed, inline frame, etc.?
Channel Help: Using [?] icon within channel… what should help text say? Who will be named as functional and technical contacts for users to request channel update?
Channel Developer Name: Who is the primary developer for this channel?
Channel Developer Email: What is the primary developer's email address?
Channel Developer Phone: What is the primary developer's phone number?
Developer Notes: Notes from the developer.
Channel Testers: Who will perform internal validation on Luminis TEST (myuatest.alaska.edu)? Who will your testers be on Luminis PREP (myuaprep.alaska.edu)?
Channel Tester Notes: Notes from the testers.
Channel Version: What version will this channel be?
Additional Comments: Are there any other comments or information you'd like to add?
Additional integration required (CPIP, LDI, etc.)? If this channel will need to integrate to an external system, how will this integration be accomplished?
